Follicular Occlusion Triad: A Case Report

Follicular occlusion triad (FOT) is a complex chronic inflammatory skin disease comprising hidradenitis suppurativa (HS), acne conglobata (AC), and dissecting cellulitis of the scalp (DCS; Hoffman’s or perifolliculitis capitis abscedens et suffodiens). While pathological mechanisms are responsible for common manifestations, exact underlying causes follicular have not yet been clearly identified. Therefore, diagnosis treatment FOT remain challenging. A 31-year-old man on conservative previously diagnosed HS AC presented to our clinic with multiple masses his posterior neck face. Excisional biopsy revealed epidermal cysts. Four months after surgery, he painful palpable mass around occipital region characteristic manifestations such as cicatricial alopecia comedones was DCS. Incision drainage lesion were performed, histopathology findings occlusion. The patient FOT. Following procedure, has regular follow-up oral isotretinoin; there no complications last 6 months.
